Occupational therapy practitioners can uniquely contribute to interprofessional care teams that address the needs of an individual throughout their lifespan, especially those with chronic conditions. Occupational therapists within the clinic can provide screening, evaluation, intervention, and education in the moment to improve successful participation in daily life and decrease the risk of decline and hospitalization.

Specific interventions provided to address patients’ primary care needs include, but are not limited to:

  • Chronic disease management and prevention
  • Medication management
  • Safety and fall prevention
  • Health promotion and lifestyle modification
  • Sedentary activity pattern modification
  • Mental and behavioral health management
  • Physical environment modification
  • Family and caregiver training and support
  • Connection to community resources
  • Driving and community mobility
  • Palliative and end of life care
