Occupational therapy practitioners can uniquely contribute to interprofessional care teams that address the needs of an individual throughout their lifespan, especially those with chronic conditions. Occupational therapists within the clinic can provide screening, evaluation, intervention, and education in the moment to improve successful participation in daily life and decrease the risk of decline and hospitalization.
